New to CLTS?
The Children’s Long-Term Support (CLTS) Program helps children with disabilities and their families through supports and services that help children grow and live their best lives.
Steps to apply
- Contact the county health and human services agency near you. Let them know you are interested in services for your child. They will help you with next steps.
- Work with the county support and service coordinator (SSC) to fill out an application, plan a home visit, and determine eligibility for CLTS Services.
- Stay connected with the SSC, and they will let you know if your child is eligible for CLTS Services.
